Health Profiles

By clicking below, you can view and print out the Personal Health History (or "Herstory" for women) that I have used in my medical practice for more than 25 years (with some adaptations of course). It includes a standard medical review plus lifestyle questions and nutritional habits, as well as stress and emotional questions. It helps us to more easily assess the general health status of our patients. Yet, even if you are not coming to the Preventive Medical Center of Marin, this will help you to take a good look at your health, and may even motivate you to make some improvements.
