Preventive Medical Center of Marin and and its entire staff are committed to helping patients obtain and maintain their optimum health.
Since 1984, PMCM has offered a wide array of Integrated Family Medicine and Preventive Health Care. Whenever possible, PMCM provides services covered by medical insurance coverage and we assist patients in billing their insurance plans.

Same Day and Next Day Appointments for acute care, such as for sore throats, ear infections, sinusitis, bronchitis, influenza, pneumonia, abdominal pain, rashes, headaches, asthma, urinary tract infections and more.
Preventive Care Services: complete adult evaluations and pre-op exams; physical exams for school and sports' women's health care and female exams and assessments for children and newborns.
Manage Chronic Problems: Asthma, High Blood Pressure, Chronic Fatique, Diabetes, Thyroid Disease, Adrenal Fatique, Food and Environmental Allergies, Irritible Bowel Syndrome and More.
On Site Services: Blood draws, Cardiovascular testing/studies, EKGs, Allergy testing (blood and skin), Heavy Metals testing, Vitality testing, Pap Smears, Suture Removal.

This is a Personal Health History Form that Dr. Haas has used in his medical practice for more than 25 years. It includes a standard medical review plus lifestyle questions and nutritional habits as well as stress and emotional questions. It helps us to more easily assess the general health status of our patients. Yet, even in the event that you are not intending to become a patient of Preventive Medical Center of Marin, this form will provide you with unique insight about your health and health habits and may even motivate you to make some improvements. Q. How does Preventive Medical Center of Marin (PMCM) differ from other medical offices?
A. PMCM practitioners are trained in multiple disciplines and are able to integrate the best and safest approaches based on each patient’s specific needs. We focus on finding the root cause of illness and doing a more natural approach to improving your health. We work together to create a plan of action towards better health. Please review the Patient Welcome Letter for other particulars or the information on our Web site.
Q. What types of practitioners are at PMCM?
A. Our family practice staff and associate practitioners include MDs, an Osteopath, two wonderful Nurse Practitioners, and an Acupuncturist that work well with children and adults of all ages. Though the specialty at PMCM is preventive medicine and health improvement, our practitioners handle most healthcare issues. Please see the Practitioners and Associates Directory and check our PMCM individual Medical Staff handouts for further information.
Q.How do I choose the right doctor for me?
A. A personal interview with the New Patient Health Coordinator provides a warm welcome to our Center. She will explain how to schedule appointments, aid in determining the right practitioner for you, review our services, and answer your questions.
Q. Do you take insurance at PMCM?
A. PMCM practitioners (not all associates, such as Drs. Shames and Rosenbaum) are contracted with various PPOs (not HMOs), such as Blue Shield, Blue Cross, United Health Care, AETNA, Interplan, Marin Foundation, and Medicare. If we are not on your list of providers, we require full payment at the time of service. Either you or PMCM can submit the necessary paperwork to your insurance for reimbursement.
Q. Will insurance cover all the services that PMCM offers?
A. Most visits and basic lab testing should be covered; however, your insurance company may consider some tests as preventive or health-based only (or not customary, since they are new) and may not fit their disease-based criteria. Sending test results with your statement may assist some people in reimbursement.
Q. What if I need lab work or prescriptions taken care of between office visits?
A. For refilled prescriptions have your pharmacy fax us the request FAX (415) 472-7636. Any other way of prescription refilling may lead to a nominal charge. Typically charges for prescriptions or lab requisitions are $35 in the Bay Area and $50 out of the Bay Area. If you need special authorization for a prescription and a review with your practitioner, there may be an additional fee for the extra time involved.
Q. What if I have an emergency?
A. When in doubt, go to the Emergency Room. If you are a patient of PMCM, your doctor can refer you to the appropriate specialist for any hospitalization care. If you need to reach the on-call doctor after office hours, the pager number is 415-721-6876. If you need phone guidance, consultation, or treatment at night or on weekends, we are happy to serve you if we are able or to refer you to an appropriate facility. There will typically be a fee charged for the practitioner’s time in caring for you (this may not be covered by your insurance).
Q. What happens when I am well and only require seasonal check-ins and a yearly cleanse?
A. Contact us and we’ll set up a special plan for you. This is a good example of Preventive Medicine. Stay Healthy!
